HOME HEALTH AIDE


Job Summary

Carries out health care tasks under the supervision of a Registered Nurse or Licensed Therapist. Provides assistance with personal hygiene, housekeeping and other related supportive tasks to a patient with health care needs.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.

-Assists patients with personal care including bed baths, oral hygiene, hair and nail care, shaving, position changing and dressing.

-Assists patients with toileting including assisting the patient on and off the bedpan, commode, or toilet.

-Assists patients with eating, measures intake and output, if necessary.

-Reading and recording temperatures, pulse, and respiration.

-Care of stable mature ostomy to include: assisting patient with care and emptying bag. Care of stable tracheostomy to include: assembling supplies, positioning patient, handing items to patient, and disposal of supplies.

-Assists patient with prescribed skin care.

-Assists patient with prescribed exercise to include: active and passive R.O.M., positioning patient, counting for patient, and increasing or decreasing activity level as defined in care plan.

-Assists patient with self-administration and reminder of medication.

-Assists patients with transferring safely from bed to chair, wheelchair or in walking with cane, crutches or walking alone.

-Accompanies patient to medical appointments only when approved by the Registered Nurse Supervisor.

-Prepares and serves meals according to instructions and patient’s care plan.

-Performs light housekeeping for the patient as indicated on the patient’s care plan.

-Lists supplies and groceries needed.

-Shops for groceries, essential household supplies and medical supplies if no other arrangement is possible.

-Performs only those tasks listed on each patient’s care plan.

-Notifies the branch office of all changes in the patient’s condition and all incidents and emergencies.

-Observation, reporting, documentation of patient status and the care services provided.

-Follows basic infection control procedures.

-Complies with dates for all in home supervision by Registered Nurse.

-Maintenance of a clean and safe environment.

-Understanding of the basic elements of body function that must be reported to the Registered Nurse Supervisor.

-Understanding of the physical, emotional, and developmental characteristics of population served by the agency, including the need for respect for the patient’s privacy and property.

-Encourages patient independence whenever possible.

-Attends 12 hours of in-service training annually.

-Submits complete and correct timesheets to the branch office on a weekly basis according to the branch office submission schedule.

-Submits all required health information to the branch office according to the submission requirements.

-May not provide services outside regular scheduled work hours.

-Uses adaptive safety equipment after competency skills reviewed and checked by Registered Nurse.

QUALIFICATIONS

EXPERIENCE

-High school diploma or general education (GED) required; completion of an approved training program in Home Health Aide services or completion of an approved competency evaluation program required.

LANGUAGE SKILLS

Ability to read and comprehend simple instructions and short correspondence in English.

MATHEMATICAL SKILLS

Ability to add, subtract, multiply, and divide in all units of measure.

CERTIFICATES, LICENSES, REGISTRATIONS

Must have successfully completed a basic home health aide training program; or have successfully passed an equivalency exam and possess written evidence of such completion.

PHYSICAL DEMANDS

While performing the duties of this job the employee is frequently required to stand. The employee must lift (adaptive-safety equipment) and/or move patients and other objects weighing as much as 50 pounds. The employee is also required to use hearing and vision during the course of work.

Must be mature and emotionally and mentally stable. Must display a sympathetic attitude toward providing services for patients at home who have medical problems.

WORK ENVIRONMENT

Licensed Practical Nurse


SUMMARY

Provides direct nursing care to patients. Teaches patients and families the aspects of the patient's care, disease process and medications.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

-Provides nursing services to patients under the direction of a registered nurse.

-Identifies and reports to the coordinating nurse, health needs of patients and families through the observation of their physical, social and emotional needs.

-Administers nursing care to patients according to the care plan.

-Contributes to the development, implementation and evaluation of care plans through conferences with the coordinating nurse.

-Reports and records the patient's condition and care, including signs and symptoms which may be indicative of change.

-Prepares clinical progress notes on the day the care is rendered.

-Submits all required paperwork weekly according to the branch office submission schedule.

-Assists the patient with the activities of daily living while teaching appropriate.

-Provides and maintains a safe environment for the patient.

-Participates in case conferences, in-service classes and other meetings as required.

-Administers medications as per physicians orders.

-Takes verbal and/or telephone orders from physicians and documents same according to agency instructions (except in Pennsylvania).

-Reports all changes, or required changes needed, in physician orders to the coordinating nurse.

-Submits all required employee health and credential information to the branch office as per company policy.

-Fulfills the companies annual in-service education requirements.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

Graduate of an approved school of practical nursing and possess a current license to practice in the state as a Licensed Practical Nurse; one year experience as a licensed practical nurse; or equivalent combination of education and experience.

LANGUAGE SKILLS

Ability to read and interpret documents and manuals. Ability to write routine reports and correspondence. Ability to speak effectively before patients and their families.

MATHEMATICAL SKILLS

Ability to add, subtract, multiply, and divide in all units of measure.

REASONING ABILITY

Ability to solve practical problems.

CERTIFICATES, LICENSES, REGISTRATIONS

Must possess a current license to practice in the State as a licensed practical nurse. Must maintain a current CPR certification.

Registered Professional Nurse


SUMMARY

Provides direct care, teaches and supervises others in aspects of patient/family care. Evaluates patient/family needs and plans patient care. Refers patients for other services as needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Obtains patient's medical history, particularly as it relates to the patient's present condition, from the patient or family member.

Performs physical assessments to identify patient needs.

Evaluates the patient's family dynamics, home situation and environment.

Evaluates the patient's condition to determine what type and frequency of services are required and obtains physician's orders for same.

Establishes, maintains, and/or revises the patient's plan of care.

Evaluates the effectiveness of the plan and makes revisions based on patient's changing needs.

Informs physician of changes in the patient's condition, plan of care and/or frequency and type of services needed.

Consults and coordinates care with other active caregivers.

Records all care provided following accepted standards of practice and using the appropriate forms.

Reports all pertinent observations and changes to the supervisor.

Interprets nursing and other services of the company to patients and families as a part of planning the patient's on-going care.

Submits all required employee health and credential information to the branch office as per Company policy.

Fulfills the Company's annual in-service education requirements.

Submits all clinical documentation weekly according to branch office submission schedule.

SUPERVISORY RESPONSIBILITIES

Instructs and supervises Licensed Practical Nurses and Aides in specific care needed to meet the goals of care.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

Graduate of an approved school of nursing; and one year of hospital experience as a Registered Nurse.

* If the RN is going to work on high-tech cases, they must have documented experience in infusion, ventilation care, etc.

Ability to read and interpret documents and manuals. Ability to write routine reports and correspondence. Ability to speak effectively before patients and their families.

MATHEMATICAL SKILLS

Ability to add, subtract, multiply, and divide in all units of measure.

REASONING ABILITY

Ability to solve practical problems.
